PATIENT CARE COORDINATOR POSITION SUMMARY The patient care coordinator provides continuity of care for the patients of our clinic, integrating care received from volunteer providers in the clinic, assistance from the health literacy team and external appointments and referrals. The patient care coordinator is responsible for ensuring quality care for patients and maintaining patient satisfaction. RESPONSIBILITIESPrimary Care Quality Measures: Recommended care by diagnosis Recommended primary care follow May assist with patient triage (both scheduled and walk-in patients)Coordination with External Providers Referral consent Referral follow up Care coordination meetings Leadership May participate in Internal Leadership Team meetings and activities Works closely with the Lead Nurse; reports to and works closely with the Clinic Administrator QUALIFICATIONS Bachelor’ s level RN preferred Oral and written English fluency required Oral and written Spanish fluency preferred Experience in family practice, internal medicine and/or with medical surgical is strongly preferred. Commitment to working with medically underserved patient populations Strong interpersonal skills and ability to communicate effectively with organizational leadership, Casa providers, patients from diverse backgrounds, and colleagues from within and outside the organization Strong, collaborative problem solving skills Ability to handle high work volume, prioritize urgent issues, and remain focused on full scope of tasks Medical triage and case management experience Good fit with a dynamic and growing nonprofit organization Working knowledge of Microsoft Word and Excel and ability to learn Electronic Medical Record System Working Conditions Work is normally performed in a typical clinic and office environment, as well as occasionally at other off-site locations Occasional evening and weekend activities Travel may be required for meetings with individuals or groups Average physical effort with some handling of light weights such as supplies or materials on an infrequent basis Benefits: This position is full-time, hourly.
